Care Coordination Services

Services for children and adolescents ages 11-19 in Washington state

In addition to individual therapy, I offer optional social work and care coordination services to provide a more comprehensive, team-based approach to supporting your child. These services are designed to complement and strengthen the progress made in therapy by enhancing collaboration between the key people involved in your child’s life.

What’s Included:

  • School Collaboration:
    I partner with your child’s school team—including teachers, special education staff, and school counselors—to align strategies and ensure consistency in your child’s support system. This may include attending special education meetings (such as IEP or 504 meetings) and providing clear, tailored recommendations to help your child thrive academically and emotionally.

  • Pediatrician Collaboration:
    I share relevant clinical insights with your child’s pediatrician to support their overall health and development, and gather important medical information that may inform my clinical assessment and treatment planning.

  • Psychiatrist Collaboration:
    For children who are prescribed mental health medications, I coordinate with your child’s psychiatrist to share relevant clinical updates and observations from therapy. This collaboration supports a more informed and cohesive treatment plan by ensuring that therapy insights are taken into account as medication decisions are made.

  • Coordination with Other Providers:
    I’m also available to work with any other professionals on your child’s care team—including speech therapists, occupational therapists, behavioral specialists, or other support providers—to ensure that everyone is working together toward the same goals.

Important Information:

  • All social work and care coordination services are optional and offered as an add-on for families currently receiving individual therapy.

  • These services are private pay only and not billable to insurance.

  • The rate is $200 per hour, prorated based on the actual time spent coordinating care.
